- Registration Certificate from MCI/NMC/J&K Medical Council /J&K Nursing Paramedical Council/other concerned relevant councils.

- Research Publications.
- One Passport-size self-attested photograph to be affixed on the application form and one spare photograph to be submitted along with the form.
